[PATCH] Refactor maia_config and system configuration page

Description

Stephan Wentz has provided a patch to RC5 which flattens the maia_config table, making it vertical rather than horizontal. This should make it much easier to add or remove system-wide configuration settings in the future. The patch includes changes for all the affected PHP scripts, but not amavisd-maia or the Perl scripts.

Due to the extreme change of files during the smarty branch, it will be triky to apply all of this, but, indeed, very important.
